4 Simple Steps to Enrollment
Fill out application form available at the Office of Admission and Aid (OAA) and attach the following requirements: Photocopy of Birth and Baptismal Certificates, Original/Photocopy of Report Card, Original/Photocopy of Certificate of Good Moral Character, 2 pcs. 1x1 ID picture
Visit OAA to get the form
Complete the admission test and interview as part of the selection process.
Schedule available upon application
A Permit to Enroll will be issued to students who passed the admission test and interview.
Wait for notification
Present the Permit to Enroll form to the Cashier's office (Window 5), fill out the Registration Form, and pay the required tuition fee based on the selected mode of payment.
Complete enrollment at cashier
Requirements by Program Level
- ✓Photocopy of Birth and Baptismal Certificates
- ✓Original/Photocopy of Report Card
- ✓Original/Photocopy of Certificate of Good Moral Character
- ✓2 pcs. 1x1 ID picture
Important Notes
- •PSA Birth Certificate: Philippine Statistics Authority authenticated copy (formerly NSO). Photocopy or certified true copy from local civil registrar may be required.
- •Good Moral Character: Must be issued by previous school, signed by principal or authorized signatory.
- •Medical Certificate: Must be recent (within 6 months) and from a licensed physician.
- •Transferees: Additional Honorable Dismissal and Transfer Credentials required.

Applications accepted for School Year 2025-2026
Scheduled Basis
Tests scheduled upon application submission. Results within 5-7 days.
May - June
Official enrollment for accepted students. Classes begin in June.
Early Application Encouraged: Limited slots available. Apply early to secure your spot and qualify for scholarship consideration.
